APPLICANT REQUESTS THAT:
His Air Force Achievement Medal (AFAM) for outstanding
achievement, rendered for the period 15 May 08 through 26 Jun
13, be replaced by the AFAM, First Oak Leaf Cluster (OLC) for
outstanding achievement for the period 26 Jun 13 through
14 Aug 13.
APPLICANT CONTENDS THAT:
When the AFAM was initially completed, the OLC was not annotated
on the Decor 6.
The applicants complete submission, with attachments, is at
Exhibit A.
STATEMENT OF FACTS:
The applicant served in the Regular Air Force in the grade of
Captain (O-3) during the matter under review.
The remaining relevant facts pertaining to this application are
described in the memorandum prepared by the Air Force office of
primary responsibility (OPR), which are included at Exhibit C.
AIR FORCE EVALUATION:
AFPC/DPSID recommends granting the applicants request to remove
the AFAM (15 May 08 through 26 Jun 13) and replace it with the
AFAM (1OLC) for outstanding achievement for the period 26 Jun 13
through 14 Aug 13. A thorough review of the applicants record
revealed that he was awarded the AFAM (basic award) on 26 Nov 03
for the inclusive period 14 Dec 01 through 14 Nov 03. The
applicant was then awarded the AFAM (should have been AFAM
(1BOLC)) for the period 15 May 08 through 26 Jun 13 for
meritorious service. However, it appears the approval authority
recognized the error (omission of First Oak Leaf Cluster) and
attempted to correct the issue; however, they did not follow
proper procedure by completing a corrected copy to change the
original decoration. Instead, the approval authority completed
a brand new certificate but kept the original special order
information. While the decoration now correctly reflects the
applicants First Oak Leaf Cluster, the period of service (15
May 08 through 26 Jun 13) is erroneous.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the
applicant on 28 Jul 14 for review and comment within 30 days
(Exhibit D). As of this date, no response has been received by
this office.
THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. After a
thorough review of the evidence of record and the applicants
complete submission, it appears as though his Air Force
Achievement Medal (AFAM), first oak leaf cluster (OLC), was
issued without First Oak Leaf Cluster annotated on the
certificate. It also appears that this award was intended to be
rendered for outstanding achievement for the period 26 Jun 13
through 14 Aug 13, to recognize the applicants contributions to
an accident investigation board, but was instead erroneously
issued for meritorious service for the period 15 May 08 through
26 Jun 13. We note the comments of the AFPC/DPSIDR indicating
that the approving official attempted to remedy this by issuing
a new certificate indicating that it was the First Oak Leaf
Cluster, but that said correction did not rectify the errors
regarding the condition for the award, meritorious service
versus outstanding achievement, or the award period. Therefore,
we agree with AFPC/DPSIDR that additional corrective action is
warranted and recommend the applicant's records be corrected as
indicated below.
THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to the APPLICANT be corrected to show that the
Air Force Achievement Medal (AFAM), rendered for meritorious
service for the period 15 May 08 through 26 Jun 13, be declared
void and removed from his records, and replaced with the AFAM,
first oak leaf cluster (1OLC) for outstanding achievement for
the period 26 Jun 13 through 14 Aug 13.

APPLICANT CONTENDS THAT: In preparation for the Air Force Reduction in Force Boards, she reviewed her records and noticed that two of her AFAMs reflect First Oak Leaf Cluster. They may not be awarded Defense/Joint decorations. The JSCM will not be awarded for any period of service for which a Military Department medal is awarded.
